Abstract Background: Sustained hypotension, cardiogenic shock, and heart failure all imply a poor prognosis in acute myocardial infarction (MI). We assessed the benefit of adding 48 hours of intra-aortic balloon counterpulsation (IABP) to standard treatment for MI, in an international trial among hospitals without primary angioplasty capabilities.
Methods: We randomized 57 patients with MI complicated by sustained hypotension, possible cardiogenic shock, or possible heart failure to receive either fibrinolytic therapy and IABP or fibrinolysis alone. The primary end point was all-cause mortality at 6 months.
Results: In all, IABP was inserted in 27 of 30 assigned patients a median 30 minutes after fibrinolysis began and continued for a median 34 hours. Of the 27 patients assigned to fibrinolysis alone, 9 deteriorated such that IABP was required. The IABP group was at slightly higher risk at baseline, but the incidence of the primary end point did not differ significantly between groups (34% for combined treatment versus 43% for fibrinolysis alone; adjusted P = 0.23). Patients with Killip class III or IV showed a trend toward greater benefit from IABP (6-month mortality 39% for combined therapy versus 80% for fibrinolysis alone; P = 0.05).
Conclusions: While early IABP use was not associated with a definitive survival benefit when added to fibrinolysis for patients with MI and hemodynamic compromise in this small trial, its use suggested a possible benefit for patients with the most severe heart failure or hypotension.
Abbreviated Abstract. We assessed the benefit of adding 48 hours of intra-aortic balloon counterpulsation to fibrinolytic therapy among 57 patients with acute myocardial infarction complicated by sustained hypotension, possible cardiogenic shock, or possible heart failure. The primary end point, mortality at 6 months, did not differ between groups (34% for combined treatment versus 43% for fibrinolysis alone [n = 27]; adjusted P = 0.23), although patients with Killip class III or IV did show a trend toward greater benefit from IABP (39% for combined therapy versus 80% for fibrinolysis; P = 0.05).

Michigan's Future
While Michigan's future appears bleak to those looking in from the outside, we in Michigan know that we can turn the state around. It will take hard work, dedication and sacrifice. Employment

Computer Works of Monroe, Inc
Business Development
For the last several years Jean has been working side by side with her sons Robert, Kevin and Matthew. Together they have built the largest technology company in Monroe.

Thomas Jeffery Bilan & Associates
Applications Engineer and WAN Technician for GMAC.

Registered Nurse
Prior to entering the challenging field of technology, Jean spent more than 10 years providing care to the members of our community. As an Intensive Care Nurse she faced daily the challenge of providing the best possible care in a system with dwindling resources and funding. In an effort to bring the promising new drugs and treatments, she accepted an additional position as the site coordinator for an international drug research study. This allowed for patients to receive earlier access to research drugs. The community also invited Jean into their homes as a home health care nurse. Jean continues to utilize her skills and training as a nurse in her roll as a Court Appointed Special Advocate, becoming the voice of an abused/neglected child in the judicial system.

The Issues
Jobs

As a small business owner and business developer for Computer
Works Inc., I am confronted with the challenges our businesses face
in the state of Michigan.

Our current legislation discourages businesses from moving into Michigan and bringing new jobs. I'll work to create an environment that encourages businesses to operate in the state of Michigan.

Michigan businesses are heavily invested in manufacturing. I will work towards strengthening our manufacturers and secure a more diversified business structure focusing on technology and alternative energy for the future.

As we enter into a global economy and move towards advanced fields of technology, we must work to further the education of our current and future workforce.

Our colleges and universities train many of the world's finest. We must work hard to retain this talent by providing a promising future.

Health Care

Health Care Insurance has become increasing unaffordable placing a tremendous burden on those who go without. Most often they are the young who are healthy whose premiums we depend on to offset the cost of insurance for those who are not in good health. We are now living longer and as every day passes we are becoming less able to care for our seniors. One third of all working Baby Boomers now assume the financial burden of older family members while working hard to save for their own retirement. Health Care is the single issue that transcends all other issues we face. Rising cost for the employers result in dropped coverage. A significant number of businesses pay far more in health care than the
Single Business TAX.

As a registered nurse, I know the importance of obtaining affordable health insurance, prescription drug coverage and promoting healthy lifestyles.

I will work to secure affordable health insurance
and prescription drug coverage for all the residents of Michigan and promote healthy lifestyle changes.

Abortion

Jean Dahm is pro-life.

Budget & Taxes

Jean Dahm believes in fiscal responsibility. She knows the importance of a balanced budget and believes in cutting wasteful spending not raising taxes. She has extensive experience balancing a corporate budget which has led to significant growth and revenue increases.

Education

As a parent of three grown sons, Jean Dahm believes in the importance of investing in education and supporting our schools.

Gun Issues

Jean Dahm believes in supporting the 2nd amendment. As a member of a military family she knows the importance of defending the 2nd amendment.
